Information you choose to submit

The admissions form asks for your name, phone number, email, city and state, birthday, ethnicity (including a “Prefer not to say” option), transportation needs, reason for applying, and any requested probation or parole information. It also records your acknowledgement of the program terms, their version, the time received, and an application reference.

Some answers can be sensitive. Please provide only the requested information. Do not include Social Security numbers, payment details, medical records, or unrelated personal information. You can call 817.683.7148 to discuss the application before sending it.

How the application is used

The website sends your answers to the ministry's admissions system so the ministry can review the application and contact you about next steps. A confirmation on the website means that system accepted the application; it does not mean you have been admitted or that a staff member has already reviewed it.

If delivery cannot be confirmed, the application may still have arrived. Keep the reference shown and call us before sending it again. Delivery events record that reference and the outcome to help investigate problems; the website's application delivery logs do not include your answers.

Call 817.683.7148 to ask what application information the ministry holds, who can access it, how long it is retained, or to request a correction or removal. The ministry can discuss what is possible for your record. Please do not put sensitive application details in a public comment or social media post.

Website hosting and donations

The website is hosted on Vercel. Hosting services may process technical request information when you visit, such as an IP address and browser information. See Vercel's privacy notice for its practices.

Donations open a separate Square checkout. Payment information is entered there, not in this website's admissions form. Square's handling of that information is explained in Square's privacy notice for customers.
